PCAP YEAR END REPORT FORM

All educators on the PCAP matrix must complete AND HAVE YOUR ADMINISTRATOR SIGN your Year End Report Form. This completed form is due in the Personnel Office on or before Monday May 21, 2012. Failure to complete and submit your Year End Report Form could result in no advancement of your salary on the PCAP salary matrix for next school year.
